For men of any age, “low T” or low testosterone levels, can wreak havoc on the body and mind. From low sex drive, impotence, poor energy, and difficulty concentrating to depression, this hormone helps to regulate a man’s body in more ways than you may have originally thought.
In cases where low T is thought to be present, the only way to confirm that the symptoms are caused by low testosterone is to have a blood test done. A specialist should be seen immediately, such as an endocrinologist, urologist, or internist.
These specialists have more experience in treating the symptoms of low T than do other types of doctors. They will also have the expertise needed to determine if something else is going on.
For men who have low T and don’t even know it (they aren’t experiencing any symptoms), there is no need for alarm. A slightly low level of testosterone may turn up in a blood test unexpectedly and the patient didn’t even know there was an issue.

- Implants placed under the skin (pellet form, slow release)
- Mouth patch (for timed absorption through the gum tissue)
- Skin patch (similar to a nicotine patch, testosterone is time released directly into the skin)

- Alcohol abuse
- Age-related hormonal changes
Very rarely, a testicular tumor, kidney disease, or liver failure are reasons for high estrogen levels in a man’s body. If a person notices enlarged breasts or glands, a trip to the doctor is the very best first course of action.
When Gynecomastia Won’t Go Away
In some cases, patients wait in vain for 2-3 years or longer for their breasts to shrink down to their former size. For these patients, surgical correction of the extra breast tissue is the only permanent and complete remedy.
In the most advanced form of surgery available today, laser-assisted liposuction is used to melt away the extra fatty tissue. This form of liposuction destroys and removes fat cells so that a normal amount remain.
Glands cannot be removed with the liposuction method; only manual excision is possible. However, a discreetly placed incision is made around the areola so that it is virtually unnoticeable after surgery.

As a cosmetic physician, I see patients with a variety of issues and factors that make their particular case unique and individual. No two people are alike – and no two surgical procedures are either!
The other day, I had a patient come in after having received male breast reduction surgery from a prominent, local plastic surgeon in NYC. The surgeon and the patient will go unnamed, however, the situation is something all too common that I must make a comment on.
Crater Deformity and Revision Gynecomastia Surgery
This man presented with what is called a “crater deformity” in the breast. This deformity is apparent when a previous gynecomastia surgery has been done improperly. Generally, the issue at hand is too much glandular tissue has been removed from the breast. In some cases, the deformity can also occur when too much fatty tissue has been removed, leaving a deep depression in the fatty tissue.
This particular patient was not a candidate for revision surgery, which broke my heart. His situation was so dire that the breast appeared to have missing muscle tissue. In common (less severe) cases of a crater deformity, it is a feasible solution to remove fat from another part of the body using tumescent liposuction and then transferring that fat to the crater deformity. The fat provides a natural cushion to fill in the area so that a natural contour can be achieved.
Unfortunately, it was not a solution for this particular patient. It was my assessment that the previous surgeon had removed so much glandular tissue that actual muscle had been removed as well. The crater deformity was made worse when the patient’s breast was flexed. In these cases, a muscle transplant (a very risky procedure with spotty success) is the patient’s only option. Since I do not perform those types of procedures due to the low success rate, I had to turn this patient away.

First off, let me say that breast cancer in men (MBC, or male breast cancer) is a very uncommon occurrence. Statistics stated by BreastCancer.org quote about 1 in 1,000 men will develop cancer in one or more breasts. The numbers for women are much high with 1 in 8 developing breast cancer during their lifetime.
The good news, aside from the fact that MBC is rare, is that it is very treatable if caught early. In this story by the Wausau Daily Herald, William (Joe) Clark was one of the unlucky men to develop breast cancer. The warning signs of breast cancer include a lump somewhere in the breast tissue or nipple, redness, puckering of the skin, discharge from the nipple, and soreness. For Joe, the only sign that something was amiss was some blood-tinged discharge from his nipple. His wife insisted that he see a doctor and he received his diagnosis.
Thankfully for Joe, the cancer was caught early and treatment options were available to him in order to be cancer free. But what about gynecomastia? Is there a set of signs that can determine whether a breast issue is cancer or gynecomastia? Yes and no.
There are similarities in breast cancer and gynecomastia in that there can be a lumpy appearance to the nipple with gynecomastia as the breast glands grow underneath the nipple. This can cause puffiness, firmness in the nipple and a visible “lump” or nodule of tissue in the nipple area. Another similarity is tenderness of the breast or nipple. If this occurs, again, please see a doctor right away for a physical exam.
There are dissimilarities in breast cancer and gynecomastia in that discharge from the nipple is NOT a sign of gynecomastia and should be checked out right away by a qualified doctor. Another major difference between cancer and gyno is that the breast tissue in men with gynecomastia will get larger in many cases, taking on a more female-like appearance (which is why they are often referred to as “man boobs”). With breast cancer, the fatty tissue of the breasts is not altered in size or volume.
The bottom line is that you need to address your health issues right away, no matter how much trouble it is to get out to the doctor’s office or however embarrassed you might be regarding where the issue is occurring on your body. Take care of yourself and stay healthy!

Why Their Before and After Pictures Don’t Tell the Whole Story

Why Pills Can’t Cure True Gynecomastia
True gynecomastia is caused by a hormonal imbalance which leads to enlarged fatty tissue and or breast glands. Just as you cannot take a pill to reduce the size of your buttocks, arms, or legs, you cannot take a “magic pill” that will reduce the size of your breasts. Especially when it comes to glandular tissue growth, the tissue that is made up of dense, glandular cells, cannot simply be whisked away by swallowing a pill. Once the tissue is present, it must be removed with gynecomastia surgery.
How to Prevent Man Boobs from Coming Back After Gynecomastia Surgery
In order to stop breasts from growing again after surgical treatment, it is imperative that patients do the following:
- Have your blood tested prior to surgical treatment to see if an underlying disease is present that may be causing the gynecomastia.
- Test your blood for testosterone and estrogen hormone levels and make sure your primary care physician has you on a plan to get your levels balanced out.
- Exercise at least 3 times a week to keep excess body fat at bay. Estrogen is stored in body fat; higher body fat levels are associated with a higher incidence of estrogen hormone imbalance in men.
- Eat well so that you can maintain a normal body weight.
- Incorporate weight lifting into your workout routine to keep muscles toned and to tighten the skin on the chest.

Many of our patients come to us having done some research about “man boobs” as they are very affectionately called. Unfortunately, there’s nothing charming about a condition like this that causes the male breasts to enlarge and take on a more female-like appearance.
But what causes this to happen to some men while others go unscathed their entire lives? The answer is that several different causes and factors go into whether or not a man develops gynecomastia. Here are the top 3 causes.
Yes, hormones are the number one reason for man boobs to develop. This is because hormones play a direct role in a variety of functions in the body, including breast growth. In gynecomastia, men have too much estrogen in their body which causes the uber-growth of male breast tissue.
This type of growth can be fatty tissue and or breast glands. Either way, men don’t want it! To get rid of man boobs for good, a full hormone panel (blood tests) need to be done to find out the extent of the hormone imbalance, to find out of any other condition exists (such as endocrine or liver disease), and to see if supplemental male hormone needs to be given to bring things back into balance.
In most cases of gyno, the patient is over 50 and has had it for several years (sometimes decades, depending upon what age the patient saw it). Sometimes it starts in the teen years and never goes away. Either way, the main result is a pair of breasts that look less than manly. Sometimes, the nipple can protrude and enlarge, causing irregularly-shaped areolas that are much too large.
Anabolic steroids are often times what brings in our bodybuilding patients to see us. These muscle-building substances cause the male breast tissue to develop because it converts to estrogen byproducts in the blood once it is metabolized. This is bad news for competitors and amateur bodybuilders alike. Unfortunately, the condition will not resolve on its own if breast glands have already grown.
In some cases where that has not occurred, the fatty tissue of the breast may shrink down to normal size again once the steroids are discontinued. Dr. Asare specializes in scarless fatty tissue removal and virtually scarless breast gland removal. This allows his bodybuilder patients to recover in about 3 days and not have to worry about their secret getting out.
Illicit and Prescription Drugs
Some prescription medications cause the breast tissue to develop. For some of these patients, switching medication is a possibility. Always ask your primary care doctor to give you a blood test to see why the breasts are developing and to be sure that it is the prescription medication that is causing the issue. If you are taking medications for your liver, prostate, HIV/AIDS, or hypothyroidism (just to name a few), it may be your medication that is causing the problem.
Marijuana, methadone (used for the treatment of drug addiction), alcoholic beverages, heroin, and amphetamines all can cause male breast tissue to grow. Men who have taken any of these substances and are experiencing breast growth should:
- Stop taking the substance (with methadone treatment, get your doctor’s approval first before discontinuing)
- Wait about 6 months to see if the breast tissue reduces on its own
- If it doesn’t go away, see your doctor to get a blood test to see if your hormone levels are out of balance
